If a tumour grows on the first scan, immunotherapy is definitely failing?
True progression is more common than "pseudoprogression". In some stable patients, further assessment may clarify an uncertain early scan, but suspected progression must not be dismissed.

Families have often read that tumours can appear to grow before shrinking on immunotherapy, and may hope that is what is happening.
What the evidence shows
Occasionally, immune cells gathering in a tumour make it look larger on an early scan, before it shrinks; this is called pseudoprogression. But it is uncommon. In most cases, growth on a scan means the cancer is truly growing. If a person is well, the team may sometimes repeat the scan a few weeks later to be sure, but they will weigh this carefully against the risk of delaying a change in treatment.
What you can do
Discuss scan results with your team, and ask how they are interpreting them. Report any worsening symptoms promptly.
