Tobacco, alcohol and vaccines

This page is the one place for the detail on tobacco, areca nut, alcohol and the two vaccines that prevent cancer. It explains what each does to cancer risk, what quitting achieves and what families can do.

The short answer

Tobacco and alcohol are two of the biggest cancer risks that families can reduce themselves, and two vaccines prevent cancers caused by viruses (cervical and liver cancer).

Tobacco and areca nut

Tobacco in any form causes cancer: cigarettes, beedi, gutka, khaini, zarda, betel quid with tobacco. IARC classifies both tobacco smoking and smokeless (chewed) tobacco as carcinogenic to humans (Group 1).

In India more adults use smokeless tobacco than smoke: about 25% and 13% in recent national estimates.

Areca nut (supari) on its own, even without tobacco, causes mouth cancer, and IARC classes betel quid as Group 1 with or without tobacco. In a multi-centre Indian study, people who chewed tobacco with areca nut had roughly double the risk of cancer of the cheek lining compared with people who chewed tobacco without areca nut.

Other people’s tobacco smoke also causes lung cancer; details are on the Pollution and cancer page.

What quitting does

Quitting helps at any age. Some benefits, such as easier breathing and a healthier heart, start within weeks or months, but cancer risk falls gradually.

Within months: easier breathing and a healthier heart.

5 to 10 years: the risk of mouth, throat and voice-box cancer is cut in half.

10 years: the risk of lung cancer is about half that of someone who still smokes.

Even after a cancer diagnosis: lower risk of a second cancer and better healing after treatment.

Studies of people who already have cancer point the same way. Among 517 current smokers with early-stage lung cancer, five-year survival was about 61% for those who quit after diagnosis and about 49% for those who kept smoking.14 In a small retrospective study of 134 people with head and neck cancer who smoked at diagnosis, those who quit before treatment were about 3.7 times more likely to have a complete response to first-line treatment.15 These are observational studies, but they show it is never too late to quit.

An Indian study showed that the risk of cancer of the cheek lining falls gradually after people quit chewing tobacco. The extra risk from areca nut, however, persisted long after quitting, which is why areca nut must be given up too.

Quitting is hard, but help is available. Ask your doctor; the free National Tobacco Quitline is 1800-11-2356.

Alcohol

IARC classifies alcohol as carcinogenic to humans (Group 1). It is linked to these seven cancers:

Mouth

Throat (pharynx)

Voice box (larynx)

Food pipe (oesophagus)

Liver

Colon and rectum

Breast (in women)

The more you drink, the higher the risk. WHO and IARC say that no safe amount of alcohol can be defined for cancer.

A WHO statement says that any drink containing alcohol, whatever its price or quality, carries a cancer risk.

About 70 to 80% of new head and neck cancers (mouth, throat and voice box) are linked to tobacco and alcohol use, and smokers have about ten times the risk of people who have never smoked.13

After cancer, alcohol can also raise the risk of the cancer coming back; see Eating well after cancer.

Vaccines that prevent cancer

Within 24 hours of birth: hepatitis B

At 14: HPV

As an adult: cervical screening

Cervical cancer is caused by the HPV virus. The vaccine protects best when it is given before the virus is caught.

India launched its national HPV vaccination programme on 28 February 2026: a free single dose of Gardasil-4 at government health facilities for girls who turn 14, on a voluntary basis. After the first campaign phase it is planned to continue through routine immunisation sessions.

WHO recommends one or two doses for girls aged 9 to 14. Ask your doctor or local health worker which vaccine and when is right for your daughter.

Gardasil-4 protects against HPV types 16 and 18 (the main causes of cervical cancer) and 6 and 11, not against every cancer-causing type. So vaccinated women should still have cervical screening as their doctor advises.

The hepatitis B vaccine was the first cancer-preventing vaccine. Chronic hepatitis B infection accounts for about 54% of liver cancer worldwide.

Check your child’s vaccination card for the hepatitis B vaccine. Adults who have never been vaccinated can ask their doctor.
