A mouth ulcer should be treated with ointments for a few months before anyone thinks of a biopsy?
If an ulcer, patch or lump is still there two weeks after a sharp tooth or a rubbing denture is fixed, it needs a biopsy, not more ointment.
Mouth Ulcer Not Healing for 3 Weeks?
A short video by Dr. Haritha Chiramana on this topic. The full answer, with the evidence, is below.

Why people believe it
Most mouth ulcers are harmless and heal, so creams, gels and antibiotics are tried again and again.
What the evidence shows
Ordinary ulcers heal within about two weeks once the cause is removed. A cancer does not heal, and every month of ointments lets it grow deeper and reach the neck glands. A small biopsy, often done in the clinic with local anaesthetic, gives the answer in days. Mouth cancer found early is cured in most people; found late it needs bigger operations and radiotherapy.
What you can do
If an ulcer or patch lasts more than two weeks, ask your doctor or dentist for a biopsy. Do not accept a third course of ointment without a diagnosis.
