Surgery offers the best chance of cure, but only some tumours can be removed safely at diagnosis. For tumours close to major blood vessels (borderline resectable), and some locally advanced ones, chemotherapy first can shrink the tumour, treat hidden spread, and show which patients will truly benefit from a major operation such as the Whipple procedure.
