Sarcoma Myth Buster #35 · Treatment

It is always better to operate first; radiotherapy before surgery only delays the cure?

Fact

For many larger sarcomas of the arm, leg or trunk, radiotherapy given before surgery controls the tumour as well as radiotherapy given after, and usually causes fewer long-term problems with the limb.

RadiotherapyHigh-energy beams, shaped by computer planning to fit the tumour and spare healthy tissue. Each session is painless and takes minutes.

Why people believe it

Everyone wants the tumour out as quickly as possible, so a few weeks of radiotherapy first can feel like a delay.

What the evidence shows

Many soft tissue sarcomas larger than 5 cm, or deep in the muscle, need both surgery and radiotherapy to stop them coming back in the same place. When radiotherapy is given first, it treats a smaller area with a lower dose, and the operation follows four to six weeks later. In a large trial the chance of cure was the same either way; radiotherapy before surgery caused more wound-healing problems at first, but less permanent stiffness, swelling and hardening of the limb. Your sarcoma team chooses the order that suits your tumour.

What you can do

Ask your team why they recommend radiotherapy before or after surgery, how to care for the wound, and which exercises will keep the limb working.
