Stage IV and palliative care Myth Buster #22 · More myths

Every patient with stage IV cancer needs ICU admission when their condition worsens?

Fact

Decisions about intensive care should depend on whether the problem can be reversed, the likely benefit and the patient's goals, not on the stage alone.

Why people believe it

When a loved one deteriorates, families often feel the ICU is the only way to show they are doing everything.

What the evidence shows

Intensive care can be life-saving for problems that can be reversed, such as a treatable infection in someone who is otherwise doing well. But when illness is advanced and the problem is not reversible, ICU treatment may prolong suffering without benefit, and separate the patient from family at the end. Discussing these situations in advance helps families make choices that reflect the patient's wishes.

What you can do

Talk with the care team, ideally before a crisis, about what treatments the patient would or wouldn't want.
