Every rectal cancer patient needs a permanent colostomy bag?
Many rectal cancers can be treated with surgery that preserves the back passage. Some patients whose tumour disappears completely after treatment may avoid surgery altogether, with very close monitoring.

What the evidence shows
For most rectal cancers, surgeons can remove the tumour and rejoin the bowel, keeping the sphincter muscle that controls the back passage. A temporary stoma is sometimes made to protect the join while it heals, and is closed a few months later. A permanent colostomy is usually needed only when the cancer is very low, involving the sphincter. For some patients whose tumour completely disappears after chemotherapy and radiotherapy, a "watch and wait" approach with strict, frequent check-ups may be possible.
What you can do
Ask your surgeon whether a stoma will be needed, whether it would be temporary or permanent, and whether watch-and-wait could be an option for you.
