If the AFP blood test goes up once during treatment, the treatment has failed?
A single AFP result does not decide whether treatment is working. Doctors look at the trend over several tests, together with scans.
Why people believe it
AFP is often the number patients watch most closely, so any rise feels alarming.
What the evidence shows
AFP is a protein made by some liver cancers. When it is high at diagnosis, a steady fall often means treatment is working, and a steady rise can be an early sign of growth, sometimes weeks before scans change. But AFP also rises with hepatitis flares and liver inflammation, and results vary a little between tests and laboratories. In many people with liver cancer AFP is not raised at all, so scans every 2 to 3 months during treatment remain the main way to judge response.
What you can do
Keep a record of your AFP results with dates and bring it to appointments. If AFP rises, your doctor may repeat the test or bring the scan forward; do not stop treatment on your own.
