Head and neck cancer Myth Buster #22 · Diagnosis & staging

A neck lump in an adult is usually TB and can be treated with TB tablets?

✕ Not true
Fact

In an adult, especially one who uses tobacco or areca nut, a firm neck lump is often cancer that has spread from the mouth or throat. A needle test must come before any TB treatment.

Fine needle test (FNAC)A fine needle draws a few cells from the lump to be examined. It is quick and done in the clinic.

Why people believe it

TB of the neck glands is common in India, so many lumps are treated as TB without a test.

What the evidence shows

A fine needle test (FNAC) takes a few minutes and tells TB from cancer in most cases. Treating cancer as TB for months lets it grow. Cutting out the lump before the mouth and throat are examined can also make later treatment harder. TB treatment should start only when TB is proven.

What you can do

Ask for an FNAC of any neck lump that lasts more than two to three weeks, and for an examination of the mouth and throat by an ENT or head and neck specialist.
