A bone marrow examination is generally essential to confirm and classify myeloma, measure how many plasma cells there are, and carry out genetic risk tests.

Blood tests can strongly suggest myeloma, but the bone marrow test confirms it, shows what proportion of the marrow is affected, and allows genetic tests (such as FISH) that show whether the myeloma is standard or high risk. These results guide the choice of treatment.
