Immunotherapy for Hodgkin lymphoma is only for when everything else has failed?

Not any more. Nivolumab, an immunotherapy medicine, combined with chemotherapy (N-AVD) is now a preferred first treatment for advanced Hodgkin lymphoma, and immunotherapy-based treatment is also used at first relapse, before a stem cell transplant.

Hodgkin lymphoma cells carry extra copies of the genes for PD-L1 and PD-L2, proteins that switch off the immune cells around them. Nivolumab and pembrolizumab block this "off switch". In a large trial in people aged 12 and over with advanced disease, N-AVD kept 92 in 100 free of progression at two years, compared with 83 in 100 with an older combination, and fewer people had to stop treatment. Side effects differ from chemotherapy: thyroid problems, diarrhoea, rash or liver inflammation can occur. It is not suitable for people with some autoimmune diseases that need medicines to suppress the immune system. In India, a lower-priced nivolumab biosimilar became available in January 2026.
